The cost of application form is Rs.1,500\/- and for postal mode Rs.100\/- extra to be included as postal charge.<\/p>\n<p style=\"text-align: justify;\">The application forms will be issued from April 15th of the year of admission.<\/p>\n<p style=\"text-align: justify;\">The last date of receipt of filled in application forms is 31st May of the year of admission.<\/p>\n<p style=\"text-align: justify;\">Certificates \/ Documents to be produced along with the application:<br \/>\n1.\u00a0\u00a0 \u00a0Attested copy of age proof.<br \/>\n2.\u00a0\u00a0 \u00a0Attested copy of the mark lists of the qualifying examination.<br \/>\n3.\u00a0\u00a0 \u00a0Attested copy of community certificate.<br \/>\n4.\u00a0\u00a0 \u00a0Attested copy of nativity certificate.<br \/>\n5.\u00a0\u00a0 \u00a0Attested copy of transfer certificate from the school last studied.<br \/>\n6.\u00a0\u00a0 \u00a0Attested copy of conduct certificate from the school.<br \/>\n7.\u00a0\u00a0 \u00a02 Nos. of Color passport Size photographs and 2 stamp size<br \/>\nphotographs.<br \/>\n8.\u00a0\u00a0 \u00a0Migration certificate for other State\/CBSE candidates.<br \/>\n9.\u00a0\u00a0 \u00a0Eligibility certificate from TN. DR. M.G.R. Medical University for Other State\/CBSE candidates.<br \/>\n10.\u00a0\u00a0 \u00a0Demand Draft for Rs.1000\/- towards registration charges drawn in favour of <strong><span style=\"color: #3366ff;\">CSI COLLEGE OF DENTAL SCIENCES AND RESEARCH,\u00a0<\/span><\/strong>\u00a0 Madurai payable at Madurai.<\/p>\n<p style=\"text-align: justify;\">The originals of all the documents should be produced at the time of admission.<br \/>\nFilled in Application with all necessary certificates should be submitted to:<br \/>\n<strong>The PRINCIPAL<\/strong><br \/>\n<strong>CSI COLLEGE OF DENTAL SCIENCES AND RESEARCH<\/strong><br \/>\n<strong>129, EAST VELI STREET, MADURAI-625 001.<\/strong><\/p>\n<p style=\"text-align: justify;\">\nAny incomplete application will be rejected.
